If you have any questions or concerns regarding your insurance coverage, please contact your insurance provider for confirmation of your doctors network participation. 

Additional Fees: 

 

There is an additional $25 dollar fee for completion of forms for use outside of the patient's medical record, including:

  • Pre-Employment

  • Employment

  • Life Insurance

  • Family Medical Leave
